When a clinic tells me its leads are too expensive, the first thing I ask for is not the ad account. I ask what happened to last month's leads. How many were contacted, how fast, how many booked, and how many actually came in. Very often the answer changes the conversation, because the cheapest lead is the one you already paid for and didn't convert.
Measure the right number
Cost per lead is easy to see and easy to optimize, which is why it misleads. A campaign that produces cheap leads who never answer the phone is more expensive than one that produces fewer, better leads who book and show up. Track the full chain:
| Metric | What it tells you |
|---|---|
| Cost per lead | How efficiently ads generate interest |
| Contact rate | How many leads your team actually reached |
| Booking rate | How well your team converts conversations to appointments |
| Show rate | How many booked patients actually attend |
| Consult-to-treatment rate | How well consultations convert |
| Cost per showed consultation | The number that connects marketing to revenue |
The five parts of an acquisition system
- A clear offer per treatment. What the patient gets, why this clinic, and a reason to act now that doesn't cheapen the brand.
- Channel fit. Search captures people who already want a treatment; social creates demand for considered treatments; referrals and reactivation are the cheapest channels most clinics underuse.
- A focused landing page and form. One treatment, one promise, one action. Ask only for what you need to call back well.
- Instant capture into the CRM. Every lead lands in one place with its source, so follow-up and reporting are possible.
- A response and booking process. Covered in detail in my guide to clinic lead follow-up.
Before you raise the budget
- Every lead is contacted within minutes during working hours
- Out-of-hours leads get an instant message and a callback time
- Your team uses a structured first conversation, not improvisation
- Reminders and a reschedule option reduce no-shows
- You know your cost per showed consultation by channel
When those are in place, extra budget buys growth. Before they are, extra budget mostly buys more leakage.
Keep it compliant
Health advertising and patient data are regulated. Follow your local health authority's advertising rules, avoid outcome promises, and get clear consent before messaging patients on WhatsApp or SMS. Trust is part of the offer, and it's easy to lose.
